We are a network of family doctors working with a team of healthcare professionals that works together with you to treat and manage your health conditions.  

Our family doctors and health professionals are located in Fort Saskatchewan, Lamont, Gibbons, Redwater, and surrounding area. Alberta Heartland Primary Care Network is focused on providing expanded service delivery and enhanced access to have the PCN "go where the people are" in order to deliver the optimum health care that all Albertans deserve.

How do I get a referral?

Visit your family doctor located in our network's points of care including Fort Saskatchewan, Lamont, Gibbons, and Redwater to get a referral.

How do I schedule an appointment or register for a workshop?
Call the Fort Saskatchewan office at 780.997.0046 to schedule an appointment at any of our locations or register for a workshop. Or by filling out the form below, the PCN will contact you directly to schedule an appointment or class.
